Where can I find my ID number?
Your ID number is very imporant.
You can find your ID number:
* in your Pressure Station Location Confirmation E-mail
* on the address label of your Resource Pack.
Your unique ID number is our way of being able to identify your Pressure Station and it is essential that you include your ID number on every Monitoring and Survey Form which your return to us after Know your Numbers! Week.
If, after checking the above, you still can't find your ID number, please email the full details of the person who registered, to: kyn@bpassoc.org.uk
Please make sure that you only include the ID number allocated by the Blood Pressure Association on your forms.
TOP TIP - to save time and to avoid confusion, it is easier to add your ID number to your forms in advance of your testing activity.
When is the deadline to return Survey Forms and where should I send them?
The deadline to return Monitoring and Survey forms is Friday 7th October. Please return forms, all marked with your ID number, to:
